Connecting Nonprofits to Resources to Help Them Succeed

Here at the Community Foundation, we understand that strong nonprofits help the entire region to thrive. In addition to providing potential funding for your organization, we are here to help connect you with resources in our community to help nonprofits succeed.

Forms

Grant Proposal Worksheet: The goal of this worksheet is to answer the questions of “Who, what, why, where, when, and how?” for your proposed project. Use this form to collect your thoughts and flesh out your proposed project before you seek and apply for funding. Answer each section as it pertains to your project idea.

Granting Funds for Community Partnership Grant Program: Here you will find a list of all the Bay Area Community Foundation funds able to grant dollars to a Community Partnership Grant Program.

Grant Application Budget Form: This is our recommended form for detailing your project/program budget as part of your grant application.

Grant Final Report Form: Bay Area Community Foundation is interested in learning about the outcomes of your grant project/program and how the funds were utilized. Grant recipients, as part of accepting the award, are required to complete a report within one year of receipt of the grant. (For grants awarded before 7.1.2022)

Media Relations Packet: As a grantee, your organization is one of the Bay Area Community Foundation’s valued partners. We want to promote the successes of this community and help your organization achieve its goals. As a part of that, we wish to share your grant impact stories. We also request that you provide public announcement of your grant through Bay Area Community Foundation. Please take a moment to read through our guidelines and contact either BACF’s CEO or communications officer with any questions. We can’t wait to tell your story!
